He is survived by his son, Cody Carlisle; mother, Susan Carlisle; sister, Windy C. Fomby (Chris) and his nieces and nephews, Josh Fomby, Brandon Fomby (Loren), Audrey Moran (Jordan) and Sarah Foley (Zack). The family will have a private memorial service at a later date.
